Another important complexity arises because the transactions between drug users and drug sellers differ sharply from the textbook model. In that model, consumers pay an agreed-upon price for a certain quantity of a good of known quality, such as a gallon of gasoline of a specified octane. In contrast, retail drug markets are characterized by conventional pricing, where consumers pay $5 or $10 for “nickel” and “dime” bags (Caulkins, 2007) and avoid any haggling about price or making change. This conventional pricing has obvious advantages for illegal transactions, but it can result in poorly informed consumers since the weight and purity of the contents of the nickel and dime bags are not standardized. The discussion so far has used the basic model of supply and demand as described in any introductory economics textbook (e.g., Frank and Bernanke, 2004). The textbook model is about an ideal market with many rational and well-informed consumers and producers who buy and sell units of a homogenous commodity.

The greatest limitation, however, lies in the sample size and non-contemporaneous data collection. Our research team was not aware of Silk Road until later in the study, but we felt that it was an important data source that was worth documenting. Geographic analyses, or at least controlling for geographic variation, were deemed to be beyond the scope of the present analysis, but an area of future research. We also did not account for the site users’ history of addiction in the analyses, despite basic research suggesting that the point in the progression of addictive disease may influence willingness to pay. Connecting drug prices to behaviors and health outcomes is a direction of future research.
Since many people with an opioid dependency get their supply from their doctor, insurance can also inadvertently defray the cost of addiction. For instance, Vicodin (the brand name version of acetaminophen/hydrocodone) costs about $142 for 100 pills (or $1.42 per tablet). High prescription and street prices for these types of pills can lead people to search for cheaper alternatives like heroin and fentanyl. StreetRx is a publicly available crowdsourcing website that collects street price data in the United States for controlled substances on the black market (9). Users can voluntarily submit the street price they paid to StreetRx and compare it to what others paid to understand if they paid a fair price on the black market.

Consumption of illegal drugs is widespread globally, and it remains very difficult for local authorities to reduce the rates of drug consumption. The team used StreetRx, a street-level research site, to call for data regarding how much money site users shelled out for diverted prescription opioids in the front half of 2012. In the end, the effort netted 954 price reports from StreetRx, the results of which the researchers compared with 737 such reports gleaned from surveys of RADARS law enforcement officers, and also with 147 reports of transaction prices on the Silk Road. Studies have shown that as high as 90% of diverted buprenorphine users indicate that their motivation for using diverted buprenorphine is to reduce withdrawal symptoms and to stop using other opioids rather than to get high (as low as 4%) (17, 18). Since 1999, opioid overdoses deaths have increased by over 600% (19) and results from our study show a steady decrease in the street price of buprenorphine. This temporal trend may reflect rising rates of buprenorphine diversion through increases in black-market supply because of public need for opioid addiction treatment (20).

APredicted relative potency refers to the potency or desirability as predicted by the street price relative to morphine. It was calculated by standardizing the price per milligram for each opioid against that of morphine. These numbers do not distinguish oral from other routes of administration, nor take into account time-release mechanisms. Geometric means and 95% confidence intervals of the price per milligram were calculated for each opioid.
Colombia’s Economy

Insulin prices vary based on manufacturer and type, so prices can be hundreds of dollars per vial for patients who pay cash. It’s illegal for people who aren’t prescribing doctors to give someone else prescription medical supplies or drugs in the U.S., but huge hurdles to accessibility have led to a thriving underground trade for diabetes supplies. They say they rolled 95 percent confidence levels into the calculations that allowed them to compare per-milligram domestic drug prices in USD. The study even included a secondary analysis, a side-by-side comparison of per-milligram rates of a morphine equivalent done through equianalgesic dose conversions.
The more there is inelasticity in either supply or demand, the higher are the social costs, construed narrowly, from constraining the quantity consumed. These analyses incorporate insights from psychological studies, including certain departures from rationality, into economic models. Gruber and Koszegi (2001) reformulated the rational model to incorporate time-inconsistent preferences. Such models may presume a high level of foresight and market knowledge among consumers—a combination that rests uneasily with the high discount rates observed in empirical research (Becker et al., 1994; Chaloupka, 1991).

Our finding of a negative relationship between health insurance coverage and street price could be consistent with either hypotheses. Further research is needed to determine which pathway is more dominant in practice. Crowdsourced data provide a valid estimate of the street price of diverted prescription opioids. The (ostensibly free) black market was able to accurately predict the relative pharmacologic potency of opioid molecules. One of the main goals of legalizing marijuana was to reduce black markets for marijuana. Black markets, however, actually persist even in states where the drug is legalized.
